Make an Appointment
If you are a new patient, please contact the physician’s office to make your first appointment.
Established patients may call the physician office to make an appointment or request an appointment using our Patient Portal.
Preparing for Your Appointment
Please bring a list of current medications, medical insurance card, photo ID, payment for services, your preferred pharmacy's phone number and correct mail order pharmacy, and a completed patient registration form and a signed HIPAA form (see links below).
For all new patients, please download and print the following forms to bring in before your visit.
For new patients of Dr. Jasper and Dr. Randolph, please print, complete, and bring forms for your visit
For new patients of Dr. Drosinos and Dr. Marin, please print, complete, and bring the forms for your visit
Please give us at least 48 hours notice of cancellation. Any cancellation with less than 48 hours notice will result in a cancelation fee ranging from $50 to $200 or more depending on the appointment.
Insurance and Payment
Payment is expected at the time of service. Please be prepared to pay any copays, co-insurances or old balances owed at the time of service. We will work with out-of-network plans, submitting claims to the insurance company on your behalf for your reimbursement. We accept Visa, Mastercard, Discover, Amex, checks and cash.
Our Secure Patient Portal
NextMD® is our safe and secure patient portal that enables you to communicate with your physician online.
Using NextMD® enables you to:
- Request or change an appointment with your physician .
- Refill your prescription(s).
- Receive your test results via secure email.
- Request your personal health records.
- Communicate with your physician 's office via secure email.
